Hallucination is not a bug you can scold out of a model with a stern prompt. It comes from how a general-purpose chatbot is built, and there are four causes worth understanding before you put one in front of your customers.

It is built to always answer

A language model produces the most likely next words, not the true ones. Faced with a question it has no real answer to, it does not stop. It fills the gap with a fluent guess that reads exactly like a real answer.

The failure looks identical to success, which is what makes it dangerous.
It has no source of truth

A generic bot is not connected to your actual policies. It answers from generic training data about businesses like yours, not from your shipping page, your returns window or your real hours, so the answer sounds right and is wrong.
